Yes, she said Mayhem was the presence that lurks in the album, kind of like a persona. It's the main persona in the Disease MV.

mayhem is the crazy witch and she's haunting the album (?) but "presents ample opportunities" is not confirming anything. it's just speculation. 

But good to know she crafted a character for the album. gaga being gaga.

The author of the article wrote this. A literal journalist who interviewed and spent hours with Gaga talking about the album and listening to it. I'm not sure what's your issue with the legitimacy of this.
