It would not surprise me if their healthcare system is severely strained and so access isn't the greatest. Also Japan may be advanced technologically but ironically in some ways they are quite conservative. Female labor force participation is unusually low among developed world and adults that support LGBT is also lower than in most Nations (I think a poll a few years ago said was only 55%) A conservative culture can be the type to create issues with personal issues of choice such as homosexuality or also have more stigma on things such as mental health (one of the characteristics that was considered)... The average working male also works a lot of hours in Japan. It could be to make up for their wives not working or just a cultural thing So it wouldn't surprise me if many people in Japan are actually quite miserable. And the aging population may affect quality of healthcare But again this is my take on it. I'm not 100% sure why Japan ranked so low.
