Woman falls to her death after after bungee jump staff forget to attach safety cord

A woman fell to her death after staff allegedly forgot to attach a safety cord before she bungee jumped from a bridge in Brazil.

Maria Eduarda Rodrigues de Freitas was thrown from the Ponte do Esqueleto, known as Skeleton Bridge, in the municipality of Limeira, São Paulo, while participating in a bungee jump on Saturday, June 13

A video posted on social media showed the moment that de Freitas, 21, was carried by staff members toward the edge of the bridge and thrown over while wearing a helmet,  but was completely unattached the bridge or to a bungee rope. 

After she fell, a witness filming pointed to the rope left behind on the ground and shouted, "Guys, the rope!"

Witnesses alleged that de Freitas was thrown without a rope and fell about 40 meters (131 feet), per G1. A police helicopter was called for rescue, but de Freitas died at the scene.

A total of 6 people were brought in for questioning in connection with de Freitas' fatal fall, a representative for the São Paulo government confirmed to PEOPLE.

Three men were arrested at the scene and charged with homicide with implied malice. 

Brazilian media said two people had tried to flee into nearby woodland after her fall, but were captured with the help of a police helicopter.

A civil police delegate said the bungee group was operating without proper permission, telling Brazilian media: “It was a team there that wasn’t regulated. They didn’t even have authorisation to be there.”

I honestly will never understand aaaany of these activities. Bungee jumping, jumping from an airplane, why would anyone willingly put themselves in situation were a tiny mistake means their life ends

in any regulated place this wouldn't be possible, so sad for what happened but this oversight would not happen usually

The fact that all these people recording did not shout out or even NOTICE the rope (which is so prominently visible if you watch other videos).

The fact that the instructor who's not jumping himself was securely tied to a rope.
