I feel like it does look like her just in bad lighting. Sometimes horrible lighting can make your cheeks look gaunt and she does have high cheek bones so it can do that. Also it seems like the actually did minimal airbrushing because you can actually see her forehead wrinkles and stuff. Seems to me like she is just upset they chose an unflattering minimally edited photo tbh.
