I don't know about other artist but thanks to the Chatty Man interview, we all know that Gaga likes to pee in trash cans in her dressing room. (Not sure if she still does tho)

Most concerts have interludes and are divided into separate acts, allowing for the singer and band and dancers or any other personnel to reorganize the set, get ready for the subsequent act, change outfits etc. I'm sure if Gaga had to go to the bathroom she would then. Or she'd just make sure to go beforehand, not have a heavy dinner or not drink a lot of fluids like coffee that contribute to having to urinate frequently.

I've seen her drink water out of a straw between interludes (such as at the artRAVE live) and I'm assuming this is also to prevent her from burping while singing which happens more often than I think people think when you're performing long sets
