Pop women, especially, are under so much pressure to present a "healthy" squeaky clean image to their fans, that even what's obvious to those who know them personally or look closely, will fly right over the heads of casual fans and listeners. It's weird to think that almost every pop girl probably has a drug issue, or had one in the past and managed to move past it. She reached out to Demi after she released Sober an addict is never fully recovered though, it’s a challenge for them everyday.
