Yeah. It's the opening of an orchestra piece. The coughing is coming from the crowd watching the orchestra

nope. its from before any piece, when the musicians tune their instruments right before it starts.

it's meant to symbolize an audience at an orchestra who coughs and let's out any noise before the orchestra starts so they don't interrupt the piece later.  It's tradition, I know it sounds weird.
