This seems a bit exaggerative? 

Mariah's always been known to be over-the-top glamourous, its almost camp at times. Especially when you consider she always controls the lighting, etc. Besides, I don't think anyone, let alone a major popstar, wants to look bad especially for a music video lol

I mean, Britney herself revealed that Mariah has always known everything about lighting and angles. Honestly I won't judge her for it because she clearly knows all her good shots lol (not a bad thing imo).

Probably the byproduct of having the entire world, industry and media constantly criticise your appearance and examine your body under a microscope for decades when you already struggle with bipolar disorder and fragile mental health due to past abuse/trauma. I agree it's a sad outcome but the question "How" is easily answered.
