Lady Gaga's body double in "Disease" was dancer Lexee Smith. I'm pretty sure this scene did not take 3 days to shoot. 

There must be a lot of stuff that we didn't see. Or did they film two music videos (maybe for the second single)

I guess the three days its for the whole video and only the stepping part was at the mental hospital she talks about. They need a double for other scenes as well and I doubt they would hire multiple people instead of using the same person.
