Gaga has repeatedly stated that she hates doing promo, and even had it mandated in her re-signed contract with Interscope that she gets to choose how much promo she does. ARTPOP, as an era, has several issues which contributed to her chart positions. The lead single competed with Roar during the height of Katy's reign. DWUW featured R Kelly. By the time GUY came around, her image issues and album backlash had over taken. Joanne had the issue of a extremely weak and sonically jarring song as the lead single. Million Reasons charted high after the Superbowl, however, many people had already purchased the track, so imo that explains a gap between its peak and #1. I think that even with exposure of putting the song on top of TTH in the week of superbowl would have helped the song to go even higher than #4 in US charts.... it could have been a #3 hit!
