This is how the "health insurance system" works here in Germany:

Roughly 90 percent of the population in Germany are members of statutory health insurance and are entitled to receive benefits to maintain and restore their health or to improve their state of health.

Statutory health insurance is financed by statutory health insurance funds which, being public-law corporations, are financially and organisationally independent. They carry out the tasks assigned to them by the State on their own responsibility.

The fundamental structural principles of statutory health insurance are the solidarity principle and the principle of benefits in kind. The solidarity principle guarantees that each insured person receives the benefits from statutory health insurance which are medically necessary, regardless of their income or of the amount of premiums which they have paid and of their morbidity risks. The principle of benefits in kind ensures benefits without up-front payments on the part of the insured.

Statutory health insurance benefits are largely funded through contributions which are paid by employers and employees. The Health Fund and a uniform contribution rate for all insured persons were introduced as per 1 January 2009. What is also new since this date is the tool of supplementary premiums for individual insurance funds.

As funding organisations of statutory health insurance, the statutory health insurance funds have the right to self-government. Self-government is exercised on an unsalaried basis by elected representatives of the insured and of the employers. The Substitute Funds, where only the insured are represented in self-government, are an exception.

Large sections of the health care system are shaped by self-government via contracts that are concluded with the health care providers. One of the core tasks of the National Association of Statutory Health Insurance Funds is to create binding collective regulations on, for instance, care provided by physicians and in-patient care.

Together with the National Association of Statutory Health Insurance Physicians, the National Association of Statutory Health Insurance Dentists and the German Hospital Federation, the National Association of Statutory Health Insurance Funds forms the Federal Joint Committee which decides on the specific benefits to be included in the statutory health insurance catalogue.

@Qualle1 we're not saying the American system is better. I certainly don't think it is. We're just saying most Americans don't care. And those that would would likely say because the state is in any way involved with the money (even if it's via other larger providers), that it can never work because the government never works, nor, to some people, should it have any more power than they personally deem necessary (and healthcare would not be among those priorities).
