If you look at Gaga's character to also be a metaphor of the "innocent/rare/different". It turns into a strong political/historical message. Sadly the general public has never put much thought into why Gaga's videos appeared "weird". So stuff like this scene was probably seen as nothing but "another random scene". It may be + I get what you mean, but I really don't think people in those countries sit around watching MTV.
