It’s about the devil on your shoulder tempting you to sin and rather than running away from fear, you indulge in the temptation :pray:

Aka it’s a meta song about radical acceptance where she accepts the darkness (her indulgences, fears, the pitfalls of fame) rather than runs from it. Rather than demonizing (pun intended) those aspects of herself, she embraces the chaos (angel) to heal herself (Monsters Never Die).

She said basically it's about her relationship with her inner demons, and I always thought it sounds like from the point of view of the personal demons when they want to seduce you with promises that you'll feel better

Like drugs, alcohol, chaotic and self-destructive behaviors, they promise you to feel better from your "disease" and even experiencing ecstasy which the thought is very seductive when you're suffering in life

It could be sexual as well but imo in a self destructive way of attempting escaping your problems

Agreed, and to an extent I'd say it represents any individual who embraces their ****ed up brain instead of trying to change it. It truly is the center piece of MAYHEM along with The Beast, imo

Lemme just use this thread to officially declare Disease is her strongest song by any meaning. Any time I play it I instantly feel like I am the owner of my own life and I can do whatever I want to, no matter how hard things have been. Like it has THAT. Oh my god the outro... Chills

i mean she can attach a deeper meaning to it all she wants, it's clearly about her giving Michael the good puss. Same how PI was clearly about her breakup from Taylor Kinney.

I mean, idk why it can’t be a layered metaphor that uses seduction as a dramatic framing device/a song that operates on multiple levels. I feel like that’s kind of the case with most of her work, tbh.

it can be but i sometimes thinks she tries to add further context to songs that are easily explained on a base level. the lyrics can be deep but at it's base i think it's clearly just a song about sex really. same as Perfect Illusion is just a breakup song, Bad Romance is just a sex and love song etc she is a world builder and i love that about her.

She's a deep and philosophical person and so am I, so I feel like i can understand and explain things deeper in her work the same way it seems you can based off of your posts on the forum and stuff. But i think sometimes over simplifying things for non deep and philosophical people helps. I think both those things can be true.

These lyrics might be read as metaphor for sex, but then it becomes way too figurative for any erotic subtext. Meanwhile if you read them as a commentary on struggles with mental health, it is easier to grasp.

i'd say it pretty much still reads as sex and being horny and needing to have sex with someone tbh. But i take your point.

It can be interpreted in a bunch of ways, but it kind of feels like it’s about us constantly wanting and expecting more from her and how intense parasocial fan dynamics can get (“screamin’ for me, baby, like you’re gonna die”). It also feels like a bit of a teaser for the era. Mayhem is Gaga’s strongest era! She blew past expectations and delivered the antidote, leaving us with our eyes rolling back in ecstasy lol.

Well the lyrics are vague enough that you could project whatever meaning you want onto it if you wanted to or tried hard enough.(As some have already done here) but from the lyrics and Gaga's explanations it seems the 2 most clear meanings are about ones inner darkness being in ones head tempting them to give up or stop trying to continue on and it saying it can be the "cure" to their problems or it's about how Gaga can be someone else's cure for their Disease (inner demons, negative traits, loneliness) by being with them in a relationship. Now what those inner demons are that she is battling  I'm not quite sure but that seems to be the strongest reading I've seen so far.

I don’t really see how the lyrics are especially vague lol. The meaning is basically one of two things: 1.) addressed to a lover or 2.) to herself. Not a whole lot of ambiguity going on other than the kind she pretty frequently establishes (Poker Face both operating as a song about thinking about women while having sex with men while simultaneously being meta commentary on Gaga using her image and persona as a way of gaining power through withholding in the industry).
