What if Mayhem is just a placeholder title on the site? Remember when Harlequin was released (or was about to be released I cant remember) and someone found another album name on the site code?

yeah, even though these info are in the source code it doesn't mean they're the correct info, right? all could be just placeholders (or a single name/release date), and and the image could be just a promo pic instead of the cover (given that we're not getting 'The Fame' cover shown)
