I’ve never read/seen the movie, but the internet says the phrase “CMBYN” arose *after* the two had sex and it was rooted in one character’s desires to be with the partner as much as he wanted to be his partner. 

But Lil Nas explains it well here, it’s like he’s singing the song to his younger self (Montero being his given birth name)
