They give a song the initial push - but the audience decides its fate from there depending on response 

The #1 thing Spotify likes is stability, whether that 6m streams or 2m - if you stabilise relative you'll hold a better TTHS position and get added in more playlists. Likewise the opposite happens 

Also artists initially all get 3 songs per big release - that's fair 

I think when a song gets added high, that's determined by the appeal and potential of the song based on the power of the artist. Daisies got added at #13 SL at #1 - gaga came off ASIB, Katy off Small Talk

When a song gets moved up randomly, the label have persuaded Spotify using recent events or upcoming promo that have/haven't happened - whether that be a big promo spot coming, a BLM issue or something else like the song going #1. 

Also ROM got added high but no cover. Chromatica release the week later Gave Gaga the cover And ROM #1  - that was probably decided before hand, and ROM initial TTHS debut at #3 was irrelevant to that - they just added it to big playlists like they would any song - the 'payola' element came with Chromatica 

That's probably all wrong but

These are almost certainly the major factors when deciding playlist-ing.
