The original plan was to have Jill survive at the end of 4, and Sidney suffers memory loss, and a new killer pops up years later, and Jill is trying to survive while keeping it a secret that she was the killer in 4. However the Weinstein's didn't want to break the formula of the previous films and said no, hence the ending in the hospital. In regards to Matthew Lillard, I doubt he will actually return, but originally, the script for Scream 3 had Stu still alive but in prison, orchestrating murders in Woodsboro. Technically, in the original theatrical release of Scream, Stu did not die. Sidney merely smashes him with the vase, and she runs out of the room. Dewey technically survived being stabbed in the back in the first one, and then stabbed multiple times in Scream 2.
