It seems like #TogetherAtHome was actually a series of daily concerts from artists (Chris Martin did it, John Legend, etc) before Gaga turned it into a big event. So I wouldn't say she joined the linear broadcast on April 18th (as they're calling), but the series.

Because the Together At Home movement was organized by them? The concert Gaga curated is part of this movement, the difference is that instead of each artist going live via their own Instagram/Youtube accounts, the One World concert will feature live streams from different artists in a single broadcast, which is going to be televised and hosted by the Jimmys and the other guy. As far as I can tell, Paulina's live stream won't be featured in the virtual concert on April 18th because her name isn't on the list, but I think it will be uploaded to their website (https://www.globalcitizen.org/en/connect/togetherathome/).
