Why didn't Perfect Illusion become a hit?

A big part of it, as people have said, is due to the lack of promo and late video.

The other part of it is just random as to how much and how quickly people engage with the song on the charts.

Honestly, I think it was the video.

Okay, it didn't smash before it's release. But with an incredible, Gaga visual - I think it could've been a lot bigger. We live in such a visually connected world... music is rarely enough for a song to explode. You have to have a good video - a good visual. 

I do think it is very different to what is currently on the radio, yes. But I don't think that is why it flopped. It needed to have an iconic, gaga 'statement' video for it to really take off - especially considering how long she has been out of the pop game. A song was never going to be enough.
