It was simply wear and tear that slowly built over the last months of the tour, and in the end became too big to keep going. You can see in the tour video's that she wasn't in the best shape weeks before the last show. And she also said that the injury caused her to become a bit more bloated, which was also visible (and probably part of the reason why people started calling her "fat").
