Unless she is talking about Gaga tossing her salad Madonna is implying that Gaga finally offered her the reverence she feels she deserves. Madonna is a mean girl, I don't think there will ever not be drama between the two of them. Well, the caption doesn't matter, since the picture clearly shows who is clinging to whom.
