She's singing to herself in the song, and the person singing is the part of her that is obsessed with the music industry. So the part of her that is obsessed with the music industry (Gaga) is saying to her actual self: "You're so tortured when you sleep, plagued with all your memories. You'll reach out and no-one's there. Like a God without a prayer. Screaming for me baby, like you're going to die." In other words, she can't feel piece unless she is Gaga. She can' t live without the fame. And then the music video obviously shows that as well. So I think she is psschoanalysiing herself and her obsession with chaos and extremes, which probably feels pretty vulnerable to her. I think she means in terms of its conception and form. She mentions that she tried really hard to go into the studio without preconceived ideas like concept, imagery or names. She said that in this album she let the music come first and I think that's what connects to what she means about vulnerability. What came first was not the package of a pop project. When that comes first, it creates a shield that makes it more difficult to see the real person behind the art, and this time around what came first was the music, the shield dropped, and that's vulnerable. You can see in Disease's lyrics that she let herself not overly construct it, not making it too intelligent and conceptual. It's simple and effective, and there's that moment on the video she says she believes pop songs can be that. Honestly, she's so articulate about everything that sometimes it's hard to follow exactly what she means.
