The meaning of the song is basically touching yourself while thinking about the one you love. I think the fire is merely the usual representation of passion, and the pain she's referring to is her suffering from being horny and away from the one she loves. Of course, the fact she's suffering from chronic pain may add another layer of interpretation to the song, but saying the song is mainly about that is a bit of a reach to me. In the end, thematically, it is a classic Gaga song in the same vein as Poker Face or Sexxx Dreams, that are all dealing with what comes to one's mind during a sexual act. Interesting! I think we know what the original song is about, the exact opposite of what it is.
