Is she feels actual pain in her face, how does injecting stuff in her face help?

If that's the case then it's because the fillers stop a relase of an enzym in the nerve muscles (or something like that), at least botox does, that's way they use it as a treatment. One of my friends got a treatment recently because her left eye would always twitch or shut down for peridos of time.

I have heard of botox being used for pain...

However, I think people are misinterpreting that clip. She references pain ON her face, not in her face. Little Detectives have already matched that clip with her Instagram posts from around PI era where she was having chronic pain and was in the doctors office. It probably has to do with hip pain.

I think she means intense look of pain in her face, "like she is running from a tiger". To me it seems like a reference to anxiety from her ptsd.

Whatever it is, it's manifested as chronic pain in her face... hence the injections.

“It is a rare moment when one is invited behind the curtain to witness the raw truth of an individual, even more so when that person happens to be one of the most recognizable, influential and scrutinized public figures in culture today,” said Lisa Nishimura, VP of original documentaries for Netflix. “Director Chris Moukarbel deftly brings us into Lady Gaga’s intimate world; allowing us to experience the drive, conflict, passion, complexity and deeply personal creative process behind this singular artist.”

Botox is prescribed for migraines and all kinds of facial pain. I don't know if this has anything to do with Gaga's situation, but it is a known medical treatment for certain facial ailments.

I don't think that is necessarily true. She may have pain in her face that is being treated with injections. But, solely based on this clip, it seems like the dr wants to treat her anxiety first, which is creating a look of pain in her face, hence the tiger reference.
