Well it's basically the virus hijacking the brain, and its strategy is to scramble and manage the brain's messages via the nervous system to organs and muscles in the body, thus causing the intense salivation, muscle spasms, and insanity that are hallmark symptoms of the disease. Not sure what would cause the hydrophobia, but probably relates to the virus's effect on the pituitary gland, which normally is responsible for the release and re-uptake of vasopressin, whose effect on the CNS to induce what we perceive as thirst.
