I don't know about people in other countries, but I know many Americans proudly associate with their ancestral heritage, no matter how many generations ago their ancestors immigrated to America. So if someone says "I'm Slovak" or "I'm Irish" it's not meant to be taken literally. It just shows pride in having heritage from that country. Exactly this. Many people say this meaning that they come from a family where someone at one point came from the said country, and that's what Gaga means when she says she's italian, though I think that she might possibly get an actual italian citizenship if she wants to.
