Why do people ignore Madonna's ephebophilic character?

Because the book is fool of tongue-in-cheek 'fantasies'. The book opens with "Everything you are about to see and read is a fantasy, a dream, pretend".
Whether it's appropriate to describe detailed fantasies is a whole other topic in itself. Feel free to make it.
